– Work closely within a multidisciplinary team to assess, plan, implement, and review care needs and pathways for individuals with complex mental health issues.
– Conduct both base-level and community-based work, requiring travel, so being a car driver is essential.
– Perform risk assessments and management, using your expertise to ensure both service users’ and your safety.
